Tips for SafetyNet reporting

Intermountain’s comprehensive safety event-management system, SafetyNet, is the official resource for reporting all safety events and concerns related to clinicians, caregivers, and patients.

These events can include, but are not limited to, physician and APP:

  • Needlesticks

  • Bloodborne pathogen exposures

  • Errors in patient medication administration

  • Delays in patient diagnosis and treatment

Any unexpected occurrence or process variance inconsistent with routine care or that could impact the quality of patient care should be reported.

Additional recommendations

The location you selected is where the event took place. It is helpful to mention the service lines needed for collaboration in your description because up to six can be added to the back end. 

  • SafetyNet prefers CSN numbers instead of the MRN W# when identifying patients. 

  • Physician peer review can be requested for physician clinical practice concerns by mentioning your request within the safety event report or by emailing Heather Stephens, Carol Calacino or Michelle Arnold with your concerns and the event report number.
