St. Mary’s Diabetes Discharge Preference List

The hospitalist group requested a more streamlined efficient process for e-scribing insulin & related supplies when discharging the patient with diabetes. Navigating the myriad of insulins, the various syringe volumes, needle lengths, pen needles, manufacturers, etc. was, as you are aware, a frustrating treasure hunt to say the least! Additionally,  if an insulin was prescribed that was not on the patient's insurance formulary additional backend office work for the provider & staff ensued not to mention delays in patient receipt of medication.

This "pref list" and accompanying explanation can help simplify your discharge process. We collaborated with the pharmacy regarding the universal language on permission to substitute a biosimilar insulin(s). And though this is legally allowed, the pharmacy did note, that some pharmacists may still request a new script.  Hopefully, though, those requests will be infrequent.

We hope this increases efficiency, reduces unnecessary work, and assures patients get their injectable DM medications & supplies in a timely manner.
