Resources for ordering durable medical equipment

As of Sept. 3, 2024, there is a new process to order durable medical equipment (DME) and home medical equipment (HME) with Parachute Health, our ordering partner.

For self-pay DME equipment only, a provider order is not required unless it will be billed to the insurance company (Medicare, Medicaid, Medicare Replacement Plans, Commercial insurance).

  • In this instance, a patient may pick up recommended DME from a DME storefront, order from Amazon, pick up from a charity closet, etc.

For DME billed to/paid for by the insurance company (Medicare, Medicaid, Medicare Replacement Plans, Commercial insurances), a provider order/prescription is required:

  • Only a person with prescriptive authority may order/prescribe (MD, DO, DPM, ARNP, PA, CNM).

    • DME order/script provisioning is out of scope for a caregiver lacking prescriptive authority (RN, PT, OT, SLP, RT, care manager, etc.).

  • The elements of this valid order/script are:

    • Patient Name

    • DME item description — the description can be either a general description (e.g., wheelchair or hospital bed), a HCPCS code, a HCPCS code narrative, or a brand name/model number

    • Quantity

    • Order Date

    • Treating Provider & NPI (MD, DO, DPM, ARNP, PA, CNM)

    • Provider signature

  • Face-to-face documentation (separate from order) by provider with prescriptive authority (Medicare/Medicaid)

    • The existing Home Health Face-to-Face documentation dot phrase has been modified to include DME – you can use the new dot phrase .DMEFACE2FACE
