Changes to Therapeutic Interchange at St. Mary’s

Medication Classes Harmonized This Month
Effective in Epic at St. Mary’s on Feb. 28, 2025

  • Histamine H2 Receptor Antagonists (H2RAs): No changes.

  • Codeine-Containing Medications: All codeine-containing medications will be removed from the formulary:

    • Codeine 30 mg tablet

    • Codeine 10 mg-guaifenesin 100 mg/5 mL oral liquid

    • Acetaminophen 120 mg-codeine 12 mg/5 mL oral solution

    • Acetaminophen 300 mg-codeine 30 mg tablet (Tylenol #3)

    • Promethazine 6.25 mg-codeine 10 mg/5 mL oral syrup

  • Non-Injection Opioids: Tramadol-acetaminophen 37.5 mg-325 mg tablet will be removed from the formulary.

  • Sedative Hypnotics: The following items will be removed:

    • Eszopiclone (Lunesta) 1 and 2 mg tablets

    • Ramelteon (Rozarem) 8 mg tablet

Please note that there are classes of medication that we haven't harmonized in Epic yet, but we will continue this process over the next few months. Please use Epic as the best resource for what we currently carry during this transition time.
