New process for reporting safety events

Intermountain’s comprehensive safety event-management system, SafetyNet, has expanded to the Peaks region. This new platform is the official resource for reporting all safety events and concerns related to clinicians, caregivers and patients.

You are an essential partner in this effort. The quality and safety of the care we provide always has benefitted from your vigilance and leadership, as well as your willingness to transparently report concerning trends and safety events.

These events can include physician and APP needlesticks, bloodborne pathogen exposures, errors in patient medication administration, delays in patient diagnosis and treatment, and more. Any unexpected occurrence or process variance inconsistent with routine care or that could impact the quality of patient care should be reported. 

Accessing SafetyNet

Employed physicians and APPs, and those with access to MyIntermountain:
You can access SafetyNet by clicking on "Report a Safety Concern" on your MyIntermountain homepage. The prompts will lead you through the process.

Affiliated physicians and APPs:
Affiliated physicians and APPs currently are unable to access MyIntermountain. A solution to this issue is being pursued and will be announced once it's realized. In the meantime, there are two simple workarounds that will allow you to continue to report safety events in SafetyNet.

  • Contact your department director or manager with safety concerns and ask that a report be submitted to SafetyNet on your behalf.

  • Call the Compliance Hotline at 877-393-6752 and ask that a report be submitted to SafetyNet on your behalf.

Please don’t forget to share when patient harm was prevented! Reporting near misses helps us identify trends that can improve quality and safety enterprise-wide. It also allows us to recognize physicians and APPs whose attention and quick action ensured the safety of a patient or care team member.
