Flu shots required by Wednesday, Nov. 15

To best protect the health of our patients and guests, our communities and everyone who works in our facilities, flu shots are required for all healthcare workers across our system. Anyone who provides direct patient care or who plans to visit an Intermountain Health facility at least once during flu season is required to get a flu shot and submit proof of vaccination by Wednesday, Nov. 15.

Submit your proof of vaccination

Clinicians who receive a flu shot need to submit proof of vaccination to us in one of the following ways:

  1. Option 1: Present your paperwork to your care site's Medical Staff Services office.

  2. Option 2: Email that documentation to peaks-cvoflu@imail.org and our CVO team will update the credentialing database.

  3. Option 3: Fax your documentation to Intermountain Health Medical Staff Services at 303-467-4284.

Medical and religious exemptions

We recognize there are circumstances that would cause you to decline a flu shot. Intermountain does not evaluate exemption requests from clinicians who are not employed by the system. If you feel you qualify for a medical or religious exemption, please request it directly from your own employer, group or organization through which you provide care at an Intermountain facility. You can submit your employer's approved exemption documentation to Intermountain via any of the submission options listed above.

All clinicians must submit their exemption by the deadline, Wednesday, Nov. 15. Physicians and providers who are granted an exemption will need to wear a mask at all times in Intermountain Health facilities during the flu season, which extends through March 31, 2024.

Those who do not receive a flu shot and who have not been granted an exemption by Nov. 15 will have their clinical privileges automatically suspended.
